Actualización en el diagnóstico y tratamiento de la hemorragia digestiva alta

Durante los últimos años, múltiples factores han condicionado la incidencia de la hemorragia digestiva alta no varicosa, haciendo que los especialistas mantengan el interés en su estudio. Como resultado, existen diversos medios de diagnóstico y modalidades para su tratamiento. Se realizó una revisión bibliográfica con el objetivo de relatar los principales avances en el diagnóstico y tratamiento de la hemorragia digestiva alta. Se utilizaron los servicios de la red Infomed: Ebsco, Medline, The Cochrane Librery, PubMed, Hinari, SciELO y Clinicalkey y el motor de búsqueda Google. Se priorizaron los metaanálisis y las revisiones sistemáticas. En la literatura predominó la melena como síntoma y la panendoscopía como el examen fundamental en el diagnóstico y tratamiento. La terapia con inhibidores de la bomba de protones es la más efectiva a alta dosis y, como método terapéutico, la modalidad de la inyección de epinefrina, asociado a un método térmico o mecánico. Es consenso que el enfoque de los pacientes con hemorragia digestiva alta no varicosa debe ser multidisciplinario, donde participen clínicos, gastroenterólogos, intensivistas y cirujanos.
