Spontaneous ruptures of the extensor tendons in the hand are frequent in patients with rheumatoid arthritis, especially in long-term progress, with ruptures of the fourth and fifth fingers being more frequent. This study presents an atypical case in our setting of spontaneous rupture of the extensor tendons of more than two fingers of the hand simultaneously. This is a 43-year-old female patient with a history of rheumatoid arthritis, referred from the rheumatology department, with one year of progress of her disease, who suffers spontaneous rupture of the extensor tendons of the last three fingers of the right hand at only one moment. Tenosynovectomy of the extensor tendons is carried out, as well as tendon transfer of the extensor of the index finger to the distal tendinous ends. A dynamic splint is placed two weeks later. Physiotherapy is performed and a functional hand is obtained after eight weeks.
