Rapunzel syndrome in a five-year-old girl

Rapunzel syndrome is the presence of gastric trichobezoars that spread to the small intestine. It is a rare disease, which occurs mainly in people with psychiatric disorders, women, and under 30 years of age. We present the case of a five-year-old female patient with increased volume in the left upper abdomen, complaining of abdominal pain. Her mother reports that she was not eating right, and that she presented frequent vomiting and weight loss. Since the first year of life they discovered that she pulled the dolls' hair and his own to ingest it; with follow-up by child psychiatry, a mental disorder was never diagnosed. The girl has normal intelligence quotient, and psychomotor and educational development. The physical examination revealed a tumor that ran from the epigastrium, the left hypochondrium and towards the mesogastrium. A contrasted stomach radiograph was taken. It was decided to operate on the patient with a diagnostic impression of gastric trichobezoar. A giant trichobezoar that spread to the jejunum was removed. The patient progressed satisfactorily; she began to eat and gradually gained weight. No new episodes of trichophagia have been corroborated. The case is singular because it occurs in a girl of only five years of age, without a diagnosed psychiatric disorder, or problems in cognitive development.Downloads
