Eccrine spiradenoma is a rare benign tumor originating in the eccrine sweat glands. Pain is an important symptom, which is present in most patients and helps to guide clinical suspicion. Early diagnosis is paramount, due to its potential malignant transformation, especially in cases of multiple or symptomatic lesions. This study presents the case of a 70-year-old man with a single, rounded and soft nodular lesion on his right forearm, without changes in surface color, no adhesion to deep planes, painful on pressure and friction, of one-year progress. Clinically it was interpreted as a lipoma. Surgical treatment was ordered to perform exeresis of the lesion and anatomopathologic study. The preoperative laboratory tests were within normal limits. The definitive histological diagnosis was solitary eccrine spiradenoma. The patient presented a satisfactory progress, and presently is asymptomatic and under medical follow-up.
