Unilateral hydrocephalus is an infrequent malformation that is produced by occlusion of one of the foramens of Monro. The symptomatology is not very florid and the neuroendoscopic treatment is the gold standard to be used. This study presents the case of a 19-year-old female patient with a perinatal history of high alpha-fetoprotein level, born from a normal labor, with a history of ventricular asymmetry and follow-up during childhood and adolescence, currently pregnant in her first trimester. She consulted the doctor due to a change in the characteristics of her headache, in addition to visual obscuration and transient weakness of the right hand, with total spontaneous recovery. In the imaging studies, a marked left ventricular dilation was detected. An analysis was established to determine the optimal treatment modality and the opportune moment to perform it. It was decided to put off the procedure to the end of pregnancy.
