Imaging studies are a fundamental pillar in the diagnosis and surgical programming of oral and maxillofacial neoplasias. This study presents the case of a 46-year-old female patient under endodontic treatment due to a molar abscess, with panoramic tomography that showed lithic image, with defined edges on the left branch of the mandible, involving the roots of the dental pieces 34, 35 and 36. She was referred to the department of maxillofacial surgery, with a suspected diagnosis of an odontogenic tumor. Tomographic evaluation was performed and a sample was taken for biopsy. Invasion or erosion of bone and peri-alveolar tissues, histological angio-adipose changes and fibrosis without signs of malignancy were ruled out. The entire lesion was removed, suspecting a mandibular hemangioma, without ruling out an odontogenic cyst. The final diagnosis was capillary mandibular hemangioma. A wide recession of the lesion was avoided with a rapid recovery and without complications. The patient required rehabilitation with removable prosthesis to restore the occlusion and the appearance. The patient is currently having a normal life.
