Malignant transformation of a mature cystic teratoma

Malignant transformation of a mature cystic teratoma located in the ovary is rare. Its diagnosis should be suspected when there are rapidly growing adnexal tumors in patients between 40 and 60 years of age and with mixed component imaging studies, larger than 10 cm in diameter. This study presents the case of a 48-year-old female patient with a personal pathological history of a gynecological tumor, diagnosed a year earlier. She comes to consultation due to abdominal growth, moderate diffuse abdominal pain with a progressive increase in intensity, accompanied by a feeling of fullness, malaise, weight loss and difficulty defecating and urinating. On physical examination, a painful tumor mass of approximately 20 cm, and of firm and regular consistency, is palpated in the lower hemiabdomen, left flank and mesogastrium. CT scan shows an image consistent with a solid cystic tumor lesion in relation to the left adnexus, consistent with a teratoma. Panhysterectomy, omentectomy and ganglionic resection are performed due to a mixed tumor of the left ovary. Histopathologic study confirms the diagnosis of a malignant infiltrating epithelial tumor of the epidermoid carcinoma type which is characterized by being moderately differentiated and originating in a mature cystic teratoma of the ovary. Infiltration of the fallopian tube and vascular congestion of the wall of said structure is observed. The patient died 75 days after the surgical treatment.
